1) open Microsoft Word file  (My Time and Talent)

2) fill out form using an x in the space provided

3) after completing the form go to file at the top of the page and select Save As

4) save to a location on your computer were you can easily locate

5) open  your E-Mail and compose a new E-Mail add file as attachment send
